Insurance and Billing

Insurances accepted

We currently accept most insurance plans, including but not limited to: Anthem, Humana, Bluegrass Family Health, Aetna, and Multiplan. At this time, our Medicaid panel is full but parents may complete paperwork to be on a wait list in the event that openings become available.

Co-pays & Co-insurances

Co-pays and Co-insurances are due at the time of service.

Insurance Card

Please be sure that we have a copy of your updated insurance for all the children in your family.

HUMANA patients – most in-house labs are not covered and must instead be "send out" labs. Unfortunately this may mean longer waits to have physical forms completed. Also, certain Humana plans require that the labs be performed in a Quest lab instead of our associated lab. Patients can express dissatisfaction with this policy by calling the customer service number on the back of their insurance card. We apologize for the inconvenience that this company is placing on patients.

Annual Records Update

We update our patient demographics annually, including address, phone number, insurance, etc. Please be patient during this time.

Medical Record

Please give our office at least 2 weeks notice when requesting copies of medical records. By state law, each person is entitled to one free copy of his or her records. For additional copies, there will be a $1 per page charge.

Hearing, Vision, and Developmental screens

are routinely done at the certain well checkups. Please check with your insurance company to see that this is a covered service.

Cell Phones

Please turn off your cell phone while in the office.
